In our grade level, we do a lunch detention. When the student serves detention, they have to fill out a behavior reflection and write about what they did. So we do have THAT documentation. But I love the idea of the labels. I have a mini graphing notebook that I keep right next to the the pocket chart and at the end of the day, or at the start of the next day, I record the colors in my journal. For simple infractions I don't document but when a student is on red I call home. I keep a one-subject notebook on my desk with each student's name on a sheet each. And whenever I call home or write them up, I record it in the notebook. I use a think sheet for lunch detention but I'm thinking I should have them writing more about their actions.
